Ahhh, yes, the "pgtable = NULL" confused me, so I was looking for a
"pgtable_t *pgtable" being passed instead, that we could manipulate.

Yes, you assume that if insert_pfn_pmd() succeeds, the table was
consumed, otherwise it must be freed.
